Quick answers

Bad breath while fasting: why, and what helps

A drier mouth and, on longer fasts, ketones. Both are common, and both are easy to manage

Two kinds of fasting breath

The first is a dry mouth. Most bad breath, about 80 to 85%, starts in the mouth itself, and the back of the tongue is the biggest reservoir of the bacteria that make it. Saliva keeps them in check. Morning breath happens because saliva slows down overnight and you don't drink, and when saliva runs low, odour increases 1. A long fast without much to drink is like a long morning.

The second is ketone breath. As your body burns more fat, your liver makes ketones, and one of them, acetone, is small enough to leave through your lungs. People who aren't dieting breathe out very little; the more fat you burn, the more there is 2. It's why some people notice a sweet or nail-polish smell, usually on fasts of a day or more, when ketones rise 3.

It's a common complaint. In an 8-week trial of alternate-day fasting, it was the side effect people reported most: 14% at the start and 29% at the end, though the change was too small to be sure it was real 4.

What helps

Fresher breath while fasting

  1. Drink water steadily through your fast, not only when you're thirsty.
  2. Brush your teeth twice a day. Toothpaste isn't swallowed, so it doesn't break your fast.
  3. Clean your tongue, especially at the back, with your toothbrush or a tongue scraper.
  4. Rinse with a mouthwash if you like; spit it out.
  5. An occasional piece of sugar-free gum helps saliva flow.
  6. Eat crunchy vegetables and drink well in your eating window.

The evidence for tongue cleaning and other single fixes is mixed: reviews haven't found any one of them to make a large difference on its own 1. Combined with enough water, they usually do the job. See does toothpaste break a fast and does gum break a fast for the details.

MYTH
Ketone breath means you're burning lots of fat, so the stronger the better.
FACT
Breath acetone does rise with fat burning 2, but it isn't a goal in itself and a stronger smell doesn't mean a better fast. Most of the benefits of daily fasting come from the habit, not from chasing ketones.

When to see a dentist or doctor

  • Bad breath lasts for weeks even when you eat normally, or comes with bleeding gums, a sore mouth or tooth pain.
  • Your mouth is dry all the time, even after drinking: some medication and health conditions can cause this.
  • You have diabetes and your breath smells fruity: check your blood sugar and ketones, and follow your care team's plan.

A note on health

Fasting Tracker is designed for general wellness and is not a medical device. Fasting is not right for everyone. If you are pregnant, diabetic, have a history of eating disorders or take medication, talk to your doctor first, and before any extended fast.
